With pandemic influenza cases on the rise across the country, federal public health authorities on Tuesday urged physicians to prescribe antiviral medicines to high-risk patients promptly but reminded the public that most people won't need, and shouldn't expect to get, the drugs if they come down with the flu.
The guidance is aimed at getting optimal benefit from Tamiflu and Relenza while preventing overuse, hoarding and shortages of the drugs, as was seen briefly during the spring outbreak of swine flu.
Specifically, authorities said, practitioners shouldn't wait for lab tests to confirm the presence of the novel strain of the H1N1 virus before starting antivirals in high-risk patients who show symptoms of flu. The drugs work best when started within 48 hours of illness.
"We think that [time] window is very important," said Anne Schuchat, director of the National Center for Immunization and Respiratory Diseases at the Centers for Disease Control and Prevention.
She added, however, that "the majority of adolescents and adults and most children won't need antiviral treatment and can be cared for with Mom's chicken soup at home."

Facebook, Twitter may be used to inform people about highway construction
The private developer responsible for rebuilding Northeast Loop 820 and Texas 121/183 in Tarrant County is working on a strategy that may include Facebook and Twitter to keep commuters informed about the project during an estimated five years of construction.
Work on the $2 billion project could begin as early as next year, if the Federal Highway Administration approves an environmental study as expected this fall. The job includes reconstruction of existing lanes and the addition of four toll lanes on Loop 820 and Airport Freeway. Continuous frontage roads also would be added.While commuters have anxiously awaited improvements to congested highways in Northeast Tarrant County, the complicated project is sure to cause orange barrel-related headaches for drivers, making it crucial to keep motorists informed about progress, Robert Hinkle, director of corporate affairs for the North Tarrant Express project."We’re going to have to figure out how to communicate our message, including the use of social media," Hinkle said to members of the 35W Coalition Wednesday, adding that Internet tools such as Facebook and Twitter likely will be used liberally to spread the word about lane closures and detours.The 35W Coalition, a group of business and political leaders, meets quarterly to track progress on efforts to expand roads in north Tarrant County, particularly between downtown Fort Worth, Alliance Airport and Dallas/Fort Worth Airport.Hinkle was hired earlier this year to become the local face of North Tarrant Express Mobility Partners, a group of companies led by Spain-based Cintra that was awarded the contract to expand Northeast Loop 820, Texas 121/183 and eventually Interstate 35W. Cintra has leased office space in North Richland Hills.The work is expected to be paid for with $570 million in state highway dollars, and about $1.4 billion in private investment. DALLAS (AP) — The Environmental Protection Agency says it won’t approve portions of Texas’ air-pollution permitting program that don’t meet the federal Clean Air Act.In a statement issued Tuesday, the agency announced its intentions to reject aspects of the plan. Final decisions about changes to the plan will be made soon under a recent agreement with Texas businesses.
Texas has major air-pollution problems, thanks to pollution from power plants, industry and cars. Houston and Dallas have never been within Clean Air Act requirements for ground-level ozone pollution.
The EPA and Texas environmental regulators have long been at odds over the state’s permitting process.
